Principal Software Engineer (Remote, Full-time) — Elixir / AI / Healthcare

Protera Health is an established, revenue-generating healthcare company that has spent years building and proving a best-in-class model of virtual musculoskeletal care: physician-led, multidisciplinary, intelligent, technology-enabled, and designed to treat patients the way we would want our own family members treated. Today, we serve multiple health plan and employer customers and are entering our next stage of growth. We are hiring a remote, full-time Principal Software Engineer to become the most senior full-time technical leader on our engineering team—a high-ownership, hands-on role for someone with strong Elixir/Phoenix experience who wants to build, code, architect, and shape the technical direction of a growing company with an existing product, real customers, meaningful revenue, and years of operating experience.

We are hiring a Principal Software Engineer to become the senior full-time technical backbone of Protera’s engineering organization.

This is not a narrow engineering job, an architecture-only role, or a 40-hour-per-week clock-in, clock-out position. We care about ownership, judgment, velocity, and outcomes. The right person will have founder-level autonomy, direct access to leadership, and the opportunity to build core systems that affect real patients, clinicians, and healthcare partners.

The Role

You will be our most senior full-time software engineer who can own day-to-day technical execution, write production code, and help run the show from a software standpoint.

You will collaborate with clinicians, enrollment, operations, analytics, commercial leaders, and occasionally clients or health plan partners.

This is a hands-on builder role. You should expect to code full-time, own major systems, make technical decisions, debug production issues, improve reliability, and turn ambiguous healthcare workflows into simple, durable software.

What You’ll Own

You will help build and own major parts of the Protera platform, including:

• Patient onboarding, engagement, and portal workflows

• Clinician workflows, scheduling, documentation support, and care coordination

• Enrollment, call center, and internal operations tooling

• Data pipelines, reporting infrastructure, and partner-facing analytics

• Integrations with EMRs, CRM systems, analytics tools, and healthcare vendors

• AI-assisted internal tools that improve speed, accuracy, and operational leverage

• Platform reliability, security, observability, performance, and maintainability

You may join client, payer, or partner calls when useful so you can understand the real-world problems our software must solve.

What We’re Looking For

We are looking for someone who has already operated at at Principal Engineer scope or higher, and wants to spend their time writing and shipping production code.

You should have:

• Deep hands-on experience with Elixir / Phoenix including LiveView

• Experienced in AI engineering techniques and tools

• Strong full-stack ability, including experience with API’s and backend systems, Python and modern data engineering techniques and systems

• Experience building and operating production systems used by real customers

• Strong database experience, especially PostgreSQL or similar relational systems

• Comfort with cloud infrastructure, monitoring, security, and production support

• Strong judgment about when to move fast and when correctness, privacy, compliance, or reliability require extra care

• Excellent communication with technical and non-technical stakeholders

• Meaningful healthcare technology experience in heavily compliant systems, such as HIPAA-regulated software, patient applications, clinician workflows, EMR/EHR integrations, payer workflows, claims, care management, and digital health operations

Pluses include:

Knowledge and experience with Salesforce development, GraphQL, FHIR, HL7, healthcare APIs, AWS, Terraform, Docker, Kubernetes, or AI-enabled internal tooling.
